At DLS Allied Health, our Occupational Therapists team that can deliver the services below.

Please note, all services listed are delivered to NDIS Participants with Capacity Building funding in their NDIS Plan and there are no ‘gap fees’ to the participant. Upon engaging services, we will provide you with a service agreement, to ensure you are aware of what services are being provided to you and the reasons why the services are being provided.

Ongoing Occupational Therapy

  • Meet with your OT on an ongoing basis to help achieve your goals. This can be weekly, fortnightly, or monthly, depending on your goals.
  • Ongoing appointments are flexible and we can meet you in your home, school, workplace or other community environment.

Functional Capacity Assessment (FCA)

  • Provide an overview of what services you may require enabling you to live independently, achieve your goals and improve your mental health.
  • Can be used to aide in requesting funding for your NDIS plan, ensuring you can access the services you need, for as long as you need them.

Supported Independent Living Assessment (SIL)

  • Provide recommendations on the frequency and level of support you require day-to-day, including overnight and weekend supports, where a high level of support is expected.
  • Provide an overview of what services you may require enabling you to live independently, achieve your goals and improve your mental health.
  • Can be used to aide in requesting funding for your NDIS plan, ensuring you can access the services you need, for as long as you need them.

Specialist Disability Accommodation Assessment (SDA)

  • Provide recommendations on the type of SDA to meet your housing goals, including a history of previous housing and why SDA is the right option for you.
  • Provide recommendations on the frequency and level of support you require day-to-day, including overnight and weekend supports, where a high level of support is expected.
  • Provide an overview of what services you may require enabling you to live independently, achieve your goals and improve your mental health.
  • Can be used to aide in requesting funding for your NDIS plan, ensuring you can access the services you need, for as long as you need them.

Sensory Profile

  • Sensory Profile helps determine participant’s sensory processing patterns in the context of everyday life and how sensory processing may be contributing to or interfering with the participant’s ability to participate in activities.
  • A sensory profile scoring sheet can be conducted in person or via tele-health.
  • In person/tele-health assessment times vary based on participants and can be completed across multiple appointments, if required.

Home Modifications

  • Home modification assessments will help determine changes that can be made in the home, to enable a participant to reach their goals.
  • Home modification assessments vary, depending on the participants goals.

Assistive Technology

  • Assistive Technology assessments and recommendations take a varying amount of time, depending on the level of cost, the level of risk and if a specialty or commercial solution is appropriate.
  • As part of assessing and recommending Assistive Technology, we can help arrange trials and short-term loans, where manufacturers/retailers are willing.

Employment

  • Employment assessments can help your employer understand the support you require to retain long term employment, including ensuring your safety.
  • Employment support, including visiting your workplace, can help ensure you retain your employment and feel safe at work.
