We are currently accepting intake forms for NDIS participants for:

• Occupational Therapy
• Speech Pathology

To get started at DLS Allied Health, please fill our NDIS Participant Intake Form (link below) with as much detail as possible. This information ensures we match the best practitioner to work with the participant or client.

If you or the participant you are referring has consented, please also attach a copy of the NDIS plan to the intake form.

After filling out the relevant intake form, please email it to admin@dlsalliedhealth.com.au

Once the NDIS Participant Intake Form has been sent to DLS Allied Health, our administration team will process the participant’s information and generate a Services Agreement and Privacy Consent Form to be signed and returned.

At this point, the administration team will put you or the person you are referring onto the waitlist. When an appointment with a practitioner becomes available, our administration team will contact you to book in an initial appointment.

Our administration team can provide you a wait list time/estimate upon request.
